Summary of program and services:	Who would benefit from the Complex/Palliative Care Program? Patients without a family physician who have been diagnosed with a life-limiting illness or has end-stage disease Patients with a Family MD - the NP will work with the patient's FMD to provide collaborative care What is a life-limiting illness? -Any disease process that has shortened a patient's life expectancy ie: Cancer, COPD, Heart Disease, Renal Failure, Liver Failure, Dementia, MS/ALS, etc. Patients who have been given a prognosis or 12 months or less or those patients who are not expected to surpass 12 months Patients with or without a Family MD Patients who wish to have EOL (End Of Life) care in the home and / or hospice (hospital) setting Patients and families who wish to have support and guidance through EOL (End Of Life) Patients who have pain and symptom management needs Patients who wish a team approach to care
